Policy and Application Maintenance Requirements

All forms that are referred to in the table can be located on the  resources tab of the Macquarie Life website.

3rd Party Requests No Policy owner can phone, email or write in with the request to add a 3rd party to their policy.
Address change No Signed request from the Policy Owner or an online case note from the Adviser.
Amended Offer sign off Yes Will be underwritten if new disclosure is obtained via the amended terms. All pages are required and all sections to be completed in full.
Application signatures No Only required for paper applications. Signatures are not required for the online applications as these are the responsibility of the Adviser.
Billing Date Change No Signed letter, email (if nominated on application), fax or verbal available for either the owner of the payment details or the Adviser.
Cancellation of application No Online adviser case note or direct request from the policy owner
Cancellation of Policy No Policy Owner - Signed quote/letter, phone call or email if the email address was nominated on the application form. 
Change of Adviser No Signed request from the Policy Owner, letter/email from the owning AFSL Director or letter/email from 2 consenting advisers within the same AFSL.
Change of cover - Adding benefits Yes New business application form
Change of name No Signed request and supporting certificate (Marriage certificate, Driver's licence)
Change in policy ownership No If superannuation policy - New application, nil personal statement required.
If non-superannuation policy - New application, nil personal statement required or Memorandum of Transfer.
Commission Notification No If the commission structure is not selected or is incorrect during the application phase then the adviser can amend at any stage via an online case note. If a change in rebating percentages and/or structure increases the premium payable then a signed quote is required.
Contribution type change - application phase No Adviser enters details directly into the application online
Contribution type change - Policy phase No Signed letter from the client or employer where applicable.
Credit Card expiry change No Signed letter, email (if nominated on application), fax or verbal available for either the owner of the payment details or the Adviser.
Increase in cover Yes New business application form
MAC request No MACs can be mailed to the Adviser or emailed to an adviser specific email address already on file.
Password request No Password will only be given directly to the owner of the Macquarie Access Code.
Payment detail change No During the application phase the Adviser can enter payment details directly into the application online. A Change of Payment Form is required for policies in force.
Premium Frequency Change No Due to frequency loadings changes from annual to monthly will require client sign off in any format. Alternatively, the adviser can notify via an online case note a change from monthly to annual or change the frequency in the application online if in application phase.
Premium Frequency Change - policy phase No Policy owner can phone, email or write in with the change request.
Reduction in cover No Policy Owner - Signed quote/letter, phone call or email if the email address was nominated on the application form. 
Release Medicals No Signed request from the Life Insured to release medicals to the Doctor, unless the Doctor was nominated on the application form. If the Doctor was listed on the application form medicals can be released following a request from the Adviser. As per page 38 of the Adviser Guide, Macquarie Life is not able to release any medical reports or results to the Adviser.
Rejection of Indexation / CPI No Policy owner can phone, email or write in with the change request.
TFN notification No During the application phase the Adviser can enter the TFN directly into the application online. No policy requiring a TFN will be put into force without TFN. The policy owner can also notify Macquarie directly via phone or letter.
